How To Register

There are three easy ways to register for either the Continuing Education Program or the Introductory HIV Pharmaceutical Care Specialist Certificate Program:

  1. Online

    You can register online with a credit card by clicking on Register Online.

  2. Fax

    Fax the registration form with your credit card information to: (716)645-2886.

  3. Mail

    Mail the registration form, along with your check or money order (made payable to UB Foundation, Inc.) or credit card information to:

This form may be filled out electronically, printed out, and sent in.

Registration Fee

The registration fees for these programs are:

HIV Continuing Education Program $225.00
Introductory HIV Pharmaceutical Care Specialist Certificate Program $475.00
These fees are payable by credit card or check (payable to UB Foundation, Inc.).

Refund Policy

Review carefully all components necessary for completion of this program. Once payment is processed and participants begin the program (pre-test), participants are considered enrolled and active; no refunds will be given. In the event that a participant is unable to complete the HIV CE or HIV Certificate program, written notification should be emailed to the program administrator: Marsha Nelson at mmnelson@buffalo.edu

Password

After registration in this program and payment is received, the password you chose in your registration (or if by mail or fax, the password we assign to you,) may be used in conjunction with your email address to access the instructional materials.

 

The University at Buffalo School of Pharmacy and Pharmaceutical Sciences is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education.
